At PneumaCare, we are dedicated to supporting both youth and adults across various vulnerable populations. Through comprehensive care and community involvement, we strive to meet the unique needs of each group, ensuring better health and well-being.

Homeless/At-Risk of Homelessness

  • Unsheltered Homelessness

  • Housing Instability

  • Must also have at least one complex behavioral, physical or developmental health need (such as: diabetes, TBI, Major Depressive Disorder, Substance Use Disorder, Intellectual Disability.)

 

 

 

 

Serious Mental Illness / Substance Use Disorder

  • Who are eligible to participate in: County Mental Health, Drug Medi-Cal Org. Delivery System or The Drug Medi-Cal program.
  • Must also experience one complex social factor: food insecurity, housing insecurity, history of ACES/trauma, history of recent contact with law enforcement related to SMI/SUD., former foster youth.
  • At risk of institutionalism, overdose, suicide, uses crisis services, pregnant and post-partum.

At Risk of Unnecessary ED & Inpatient Visits (High Utilizer)

Adult

5 or more ER visits and/or

3+ unplanned hospitalization admissions and/or

Multiple short-term skilled nursing facility stays

Children

3 or more ER visits and/or

2+ unplanned hospital admissions

In the past 6 months

Birth Equity

  • Are pregnant or are postpartum (within 12- month period).
  • Subject to racial and ethnic disparities as defined by California public health data on maternal morbidity and mortality.
  • Black, American Indian, Alaskan Native, and Pacific Islander.

Children & Youth Enrolled in California Children’s Services

  • Children & youth who are enrolled in CCS or CCS WCM.
  • Are experiencing at least one complex social factor influencing their health. Examples include but are not limited to: lack of access to food, lack of access to stable housing, difficulty accessing transportation, high measure (4+) of ACEs screening, history or recent contacts with law enforcement; or crisis intervention services related to mental health and/or substance use symptoms.

Children and Youth Involved in Child Welfare

Must meet one or more of the following conditions:

  • Are under 22 and are currently receiving foster care in California.
  • Are under age 21 and previously received foster care in California or another state within the last 12 months.
  • Have aged out of foster care up to age 26 (having been in foster care on their 18th birthday) in California or another state.
  • Are under 18 and are eligible for California’s Adoption Assistance Program.
  • Are under 18 and currently receiving or have received services from California’s Family Maintenance Program within the last 12 months
